Clinical trials play a prominent role today in medicine, but are not without controversy. These issues start from the day physicians begin their specialization process in medical school and continues onto their day-to-day practice as attendings with referral patterns and resulting financial incentives. This combined with the lack of training in basic issues of epistemology and statistics, allows poor interpretations of clinical trials to reign free. A proposal to integrate the notion of severity to help remedy these issues are made (...) by ensuring that studies are tested “As Severe As Reasonably Possible” will put P-values, multiplicity adjustments, and model checking in their rightful place. In light of this, multiple clinical trials are examined with an in-depth analysis of erroneous statistical inferences taken from phase III randomized trials, non-inferiority, early phase, as well as observational studies. Severe testing in the setting of medicine will help with alleviate some of the issues with the current misinterpretation of clinical studies. (shrink)

Subcreative sets, introduced by Blum, are known to coincide with the effectively speedable sets. Subcreative sets are shown to be the complete sets with respect to S-reducibility, a special case of Turing reducibility. Thus a set is effectively speedable exactly when it contains the solution to the halting problem in an easily decodable form. Several characterizations of subcreative sets are given, including the solution of an open problem of Blum, and are used to locate the subcreative sets with respect to (...) the complete sets of other reducibilities. It is shown that q-cylindrification is an order-preserving map from the r.e. T-degrees to the r.e. S-degrees. We present a one per cent measurement of the cosmic distance scale from the detections of the baryon acoustic oscillations in the clustering of galaxies from the Baryon Oscillation Spectroscopic Survey, which is part of the Sloan Digital Sky Survey III. Our results come from the Data Release 11 sample, containing nearly one million galaxies and covering approximately 8500 square degrees and the redshift range 0.2 < z < 0.7. We also compare these results with those from the publicly released (...) DR9 and DR10 samples. Assuming a concordance Λ cold dark matter cosmological model, the DR11 sample covers a volume of 13 Gpc3 and is the largest region of the Universe ever surveyed at this density. We measure the correlation function and power spectrum, including density-field reconstruction of the BAO feature. The acoustic features are detected at a significance of over 7σ in both the correlation function and power spectrum. Fitting for the position of the acoustic features measures the distance relative to the sound horizon at the drag epoch, rd, which has a value of rd,fid = 149.28 Mpc in our fiducial cosmology. We find DV = at z = 0.32 and DV = at z = 0.57. At 1.0 per cent, this latter measure is the most precise distance constraint ever obtained from a galaxy survey. Separating the clustering along and transverse to the line of sight yields measurements at z = 0.57 of DA = and H =. Our measurements of the distance scale are in good agreement with previous BAO measurements and with the predictions from cosmic microwave background data for a spatially flat CDM model with a cosmological constant. © 2014 The Author Published by Oxford University Press on behalf of the Royal Astronomical Society. FRANCIS HERBERTBRADLEY (1846-1924) 1 agreed with the other English idealists that the real world is the experienced world. But he started with the fundamental conviction that "experience" is more than "thought," as Green had maintained. Bradley's basic drive is the refusal to abolish "feeling" in favor of knowledge and intelligibility. "Feeling" is a fundamental and (...) ineradicable aspect of "reality"m more fundamental, in fact, than thinking. In the immediately experienced world, all intelligible distinctions and relations are merged in a "whole of feeling" which is the ultimate subject-matter of philosophy. "Thought" arises out of feeling, out of "immediacy," as an interpretation of immediate experience by distinguishing relations and aspects in it. "Knowledge " is only one aspect of the experienced world; it needs blending with "feeling" and will to include all the experienced aspects. "Feeling" starts for Bradley, in his Principles of Logic (1883), in pretty much the empiricist sense, as isolated sensations, and as subjective (in flavor at least). It then becomes neutral: "feeling" is identified with "the felt"; and it grows richer, attempting to include the whole wealth of immediate experience. Thus Bradley gets very close to Dewey's conception of "direct experience," "non-reflective experience," without, however, Dewey's biological analysis. The real, experienced world is never identical with the intelligible world, yet it is also never "extraneous " to it. The relation of the two can be said to be, in Aristotelian terms, that of substance to form, that of what is to its intelligible aspect. The intelligible world, the system of science and knowledge, thus fundamentally needs criticism in the light of its setting in the experienced world. BackgroundConventionally, transcranial direct current stimulation (tDCS) aims to focalize the current reaching the target region-of-interest (ROI). The focality can be quantified by the dose-target-determination-index (DTDI). Despite having a uniform tDCS setup, some individuals receive focal stimulation (high DTDI) while others show reduced focality (“non-focal”). The volume of cerebrospinal fluid (CSF), gray matter (GM), and white matter (WM) underlying each ROI govern the tDCS current distribution inside the brain, thereby regulating focality.AimTo determine the regional volume parameters that differentiate the focal and (...) non-focal groups.MethodsT1-weighted images of the brain from 300 age-sex matched adults were divided into three equal groups- (a) Young (20 ≤ × < 40 years), (b) Middle (40 ≤ × < 60 years), and (c) Older (60 ≤ × < 80 years). For each group, inter and intra-hemispheric montages with electrodes at (1) F3 and right supraorbital region (F3-RSO), and (2) CP5 and Cz (CP5-Cz) were simulated, targeting the left- Dorsolateral Prefrontal Cortex (DLPFC) and -Inferior Parietal Lobule (IPL), respectively. Both montages were simulated for two current doses (1 and 2 mA). For each individual head simulated for a tDCS configuration (montage and dose), the current density at each region-of-interest (ROI) and their DTDI were calculated. The individuals were categorized into two groups- (1) Focal (DTDI ≥ 0.75), and (2) Non-focal (DTDI < 0.75). The regional volume of CSF, GM, and WM of all the ROIs was determined. For each tDCS configuration and ROI, three 3-way analysis of variance was performed considering- (i) GM, (ii) WM, and (iii) CSF as the dependent variable (DV). The age group, sex, and focality group were the between-subject factors. For a given ROI, if any of the 3 DV’s showed a significant main effect or interaction involving the focality group, then that ROI was classified as a “focal ROI.”ResultsRegional CSF was the principal determinant of focality. For interhemispheric F3-RSO montage, interaction effect (p < 0.05) of age and focality was observed at Left Caudate Nucleus, with the focal group exhibiting higher CSF volume. The CSF volume of focal ROI correlated positively (r ∼ 0.16, p < 0.05) with the current density at the target ROI (DLPFC). For intrahemispheric CP5-Cz montage, a significant (p < 0.05) main effect was observed at the left pre- and post-central gyrus, with the focal group showing lower CSF volume. The CSF volume correlated negatively (r ∼ –0.16, p < 0.05) with current density at left IPL. The results were consistent for both current doses.ConclusionThe CSF channels the flow of tDCS current between electrodes with focal ROIs acting like reservoirs of current. The position of focal ROI in the channel determines the stimulation intensity at the target ROI. For focal stimulation in interhemispheric F3-RSO, the proximity of focal ROI reserves the current density at the target ROI (DLPFC). In contrast, for intrahemispheric montage (CP5-Cz), the far-end location of focal ROI reduces the current density at the target (IPL). (shrink)

No point of John C.Calhoun's political thought has been more disputed than exactly where it is situated in the theoretical landscape. Calhoun has been treated as the �Marx of the master class� by Richard Hofstadter; a �reactionary conservative� arguing eclectically from liberal premises by Louis Hartz; an authentic conservative by Russell Kirk, Clinton Rossiter and August Spain; and a precursor to the pluralist vision of politics by Peter Drucker. Two of the most engaging treatments of Calhoun's thought are Darryl (...) Baskin's and Peter Steinberger's, both of relatively recent vintage. Baskin argues that despite Calhoun's use of vocabulary borrowed from organic conservatism he is essentially a classical liberal and, as such, is engaged in the typically liberal �flight from community�. He has no true notion of civic virtue and his concept of the public interest is only a mechanistic sum of private interests. Steinberger agrees that Calhoun is a liberal, but argues that his contribution to American liberalism is precisely to sublate selfishness to an authentic civic virtue. These disputes are interesting, but I suggest that in the efforts to contain Calhoun within the conservative/liberal scheme of categorization some of the subtlety of his thought is lost. Too often an appreciation of his thought is sacrificed to an underlying agenda of attacking or defending one ideology or another. In the present critique I attempt to recover some insights that have been submerged in other treatments. Calhoun's constitutional theory of concurrent majoritarianism, rather than his liberal or conservative bent, is the focal point of sections III and IV. I argue that Calhoun's theory exposes -- sometimes unwittingly -- the stern limits of what constitutions can accomplish. In section V I suggest that insofar as Calhoun is generally a liberal (as Hartz, Baskin and Steinberger contend) an internal contradiction within his theory points to a paradox at the very heart of liberalism. POPKIN THE THEORY that the revival of ancient scepticism, and the application of its arguments to the controversies of the sixteenth century, played a vital role in the development of modern philosophy was first suggested by me almost forty years ago. A three-part article in the Review of Metaphysics entided "The Sceptical Crisis and the Rise of Modern Philosophy" (...) contained much of the theory.' What was added by the time of my History of Scepticism from Erasmus to Descartes (published in x96o,' but finished by 1958 ) was the importance of the sceptical arguments in the controversies of the Reformation and the CounterReformation, and the development of a religious scepticismmfideism--as a way of fending off criticism of basic religious beliefs.s One of the first reviews of my book was by Charles B. Schmitt,4 which he soon followed with his volume on Gianfrancesco Pico and then one on Cicero Scepticus.5 Before Charles Schmitt and I entered this arena, scepticism as a living philosophical view was hardly mentioned in histories of philosophy, which, by and large, saw modern philosophy as a rejection of Scholasticism, with nothing in between. There were practically no articles about or studies of scepticism as a philosophical attitude or activity in modern philosophy before we went to work. The American philosopher Roderick Chisholm had written an article on Sextus and modern empiricism, and that was about it.6 When I took my first course in the history of philosophy with John Herman Randall, I frankly could not make much out of Plato or Aristotle, but when we were assigned to read Sextus Empiricus, I found an author who made sense. Later in the course I was introduced to Hume, whom Randall hated, and I loved. In a course with Paul Oskar Kristeller a few years later, on Hellenistic philosophy, Sextus appeared as a serious figure. A couple of years after that, in a seminar with Charles Hendel on Hume, I wrote a paper on Sextus and Hume. I showed it to Kristeller, who said drily and carefully that it might be worth looking into whether Hume had read Sextus, and whether there was a prior tradition of scepticism in European philosophy. Thus was I set off on the quest. Schmitt and I not only offered a lot of evidence previously ignored or unknown about the recovery of sceptical texts and their use in the philosophical discussions of the sixteenth and early seventeenth centuries, but we also offered a new paradigm, a new way of conceiving what problems were being agitated, a new way of understanding the kinds of answers being given as ways of resolving the sceptical crisis. The reception of our theory indicated that it provided a more meaningful picture of the making of the modern mind than what had previously been proposed.7 What I should like to deal with now are three questions: Has our theory stood up over almost four decades? Limitations of space will force me to confine my remarks to only a few of these carefully researched investigations. In the first study Bormann seeks to determine Aristotle's views concerning the ultimate ground for universally valid propositions and Aristotle's justification for his belief that certain of these are valid for all men and at all times in spite of the differences in human language. This quickly leads him to consider Aristotle's views concerning mind as active and mind as passive, especially as found in De anima III, 5. Mind as active is not to be identified with Aristotle's Unmoved Mover of Metaphysics XII; it unites with the soul of the individual man during the latter's lifetime, is free from all potentiality, is never acted upon, and exists in separation from the individual human being with the death of the latter. Mind as passive passes away with the individual soul when a particular man dies. Bormann then examines subsequent efforts to clarify Aristotle's thought concerning this by Alexander of Aphrodisias, Themistius, John Philoponus, Averroes, Albert the Great, and Thomas Aquinas. Lüttringhaus introduces his discussion of Boethius's views on God, freedom, and necessity by drawing an interesting contrast between freedom as found in the One of Plotinus and freedom in Boethius. Given his Christian Neoplatonism, Boethius must reconcile human freedom with an omnipotent and omniscient God. His efforts to do so in the Consolatio are then carefully analyzed by Lüttringhaus. As Lüttringhaus points out, Boethius's distinction between absolute necessity and hypothetical necessity in Bk. V is crucial for his defense of human freedom, along with his distinction between divine eternity and human temporality. G. Vuillemin-Diem's study offers new and valuable information concerning Moerbeke's revision and translation of the Metaphysics. Contrary to the long held view that Moerbeke had only newly translated one book and had simply revised an earlier translation for the others, she shows that he newly translated Bks. XI, XIII, and XIV. Moreover, she reveals that a long overlooked surviving Greek manuscript may have served as the model for Morebeke's revision and translation, either directly or through a copy. R. Wielockx turns to three manuscripts from Godfrey of Fontaines' library which still survive in order to cast some new light on the disputed authenticity of Albert the Great's Summa theologiae. The manuscripts contain a number of excerpts taken from the first part of Albert's Summa; many of those contained in the two last-mentioned manuscripts are explicitly attributed to Albert by Godfrey's own hand, according to Wielockx. Given this and the relatively early dating Wielockx has established for the copying of these excerpts into the manuscripts in question, he has here discovered a valuable external and positive indication pointing to the authenticity of at least this earlier part of Albert's Summa. Having already published an important book in 1977 on the Paris Condemnation of 1277, R. Hissette here returns to the disputed issue whether some of these condemned propositions were directly aimed at Thomas Aquinas or at Albert the Great. Hissette acknowledges that a number of positions held by Aquinas and by Albert were in fact touched on by various condemned propositions. He also notes that the authors of the decree itself must have known that the condemned propositions were held by Aquinas and by various Masters from the Arts Faculty at Paris. But given the statement in the prologue of the decree to this effect that it was aimed at members of the Arts Faculty, Hissette continues to deny that either Thomas or Albert was a direct target of this condemnation; they were only indirectly implicated. Some may wonder whether this distinction defended by Hissette is real or only verbal. In his careful analysis of Thomas's De veritate, 1, 6, Zimmermann draws some interesting comparisons and contrasts between Thomas's views concerning whether any created truth is immutable and Anselm's De veritate, on the one hand, and Bertrand Russell's "Truth and Falsity," on the other.--John F. In a recent essay, Donald Dripps advanced what he calls a “commodification theory” of rape, offered as an alternative to understanding rape in terms of lack of consent. Under the “commodification theory,” rape is understood as the expropriation of sexual services, i.e., obtaining sex through “illegitimate” means. One aim of Dripps's effort was to show the inadequacy of consent approaches to understanding rape. Robin West, while accepting Dripps's critique of consent theories, criticizes Dripps's commodification approach. In its place, West suggests (...) a more phenomenological approach. The author argues that neither Dripps nor West offers convincing critiques of consent-based theories; the alternatives they offer presuppose the vitality of a consent-based approach to understanding rape; and that both Dripps and West consistently conflate more general moral and political issues with that of the nature of rape. While functioning quite well for many years, the bioethics profession is in crisis. John H. Evans closely examines the history of the bioethics profession, and based on the sociological reasons the profession evolved as it did, proposes a radical solution to the crisis.

A well-known open problem in epistemic logic is to give a syntactic characterization of the successful formulas. Semantically, a formula is successful if and only if for any pointed model where it is true, it remains true after deleting all points where the formula was false. The classic example of a formula that is not successful in this sense is the “Moore sentence” p ∧ ¬ BOX p, read as “p is true but you do not know p.” Not only (...) is the Moore sentence unsuccessful, it is self-refuting, for it never remains true as described. We show that in logics of knowledge and belief for a single agent (extended by S5), Moorean phenomena are the source of all self-refutation; moreover, in logics for an introspective agent (extending KD45), Moorean phenomena are the source of all unsuccessfulness as well. This is a distinctive feature of such logics, for with a non-introspective agent or multiple agents, non-Moorean unsuccessful formulas appear. We also consider how successful and self-refuting formulas relate to the Cartesian and learnable formulas, which have been discussed in connection with Fitch’s “paradox of knowability.” We show that the Cartesian formulas are exactly the formulas that are not eventually self-refuting and that not all learnable formulas are successful. The moral worth of attorneys has traditionally been judged in terms of compIiance with legal codes of ethics. These codes, ostensibly designed to promote smooth and equitable functioning of an adversary system, are manifestations of a rule utiIitarian moral system. This paper argues that ethical attorneys have a higher calling than rule compIiance and that "moral commitment," which combines commitment to "right" solutions and moral courage, is a superior yardstick for measuring their moral worth.

The two theories that revolutionized physics in the twentieth century, relativity and quantum mechanics, are full of predictions that defy common sense. Recently, we used three such paradoxical ideas to prove “The Free Will Theorem” (strengthened here), which is the culmination of a series of theorems about quantum mechanics that began in the 1960s. It asserts, roughly, that if indeed we humans have free will, then elementary particles already have their own small share of this valuable commodity. More precisely, if (...) the experimenter can freely choose the directions in which to orient his apparatus in a certain measurement, then the particle’s response (to be pedantic—the universe’s response near the particle) is not determined by the entire previous history of the universe. Our argument combines the well-known consequence of relativity theory, that the time order of space-like separated events is not absolute, with the EPR paradox discovered by Einstein, Podolsky, and Rosen in 1935, and the Kochen-Specker Paradox of 1967 (See [2].) We follow Bohm in using a spin version of EPR and Peres in using his set of 33 directions, rather than the original configuration used by Kochen and Specker. More contentiously, the argument also involves the notion of free will, but we postpone further discussion of this to the last section of the article. He argues that professionalization of occupations can both harm and benefit society, and that by converting occupations into organized special interest groups, the professions serve some sectors of society at the expense of others. Kant’s philosophy of science takes on sharp contour in terms of his interaction with the practicing life scientists of his day, particularly Johann Blumenbach and the latter’s student, Christoph Girtanner, who in 1796 attempted to synthesize the ideas of Kant and Blumenbach. Indeed, Kant’s engagement with the life sciences played a far more substantial role in his transcendental philosophy than has been recognized hitherto. The theory of epigenesis, especially in light of Kant’s famous analogy in the first Critique, posed crucial (...) questions regarding the ‘looseness of fit’ between the constitutive and the regulative in Kant’s theory of empirical law. A detailed examination of Kant’s struggle with epigenesis between 1784 and 1790 demonstrates his grave reservations about its hylozoist implications, leading to his even stronger insistence on the discrimination of constitutive from regulative uses of reason. The continuing relevance of these issues for Kant’s philosophy of science is clear from the work of Buchdahl and its contemporary reception. (shrink)
